COVID-19 Vaccinations
PFIZER VACCINATION
Currently available for anyone over 12 years of age
Second dose appointments will be minimum 3 weeks after dose one.
If you are aged over 60 and wish to have Pfizer you can now book an appointment with us.
If you are a new patient you will need to fill in the new patient form online when you book your appointment. If you book your appointment over the phone you will need to arrive early and fill this form out on the day of your appointment.
If you have not completed your online consent prior to your first Pfizer vaccine with us, upon arrival you will need to use the QR code at reception to complete the Pfizer consent form prior to your appointment , this will take approximately 5minutes. If unable to complete via the QR code you will be given a paper version of the Pfizer consent form.
ASTRAZENECA VACCINATION
We continue to administer AstraZeneca to those over 60years old
Second doses can now be administered from 6 weeks after dose 1, however for best COVID-19 immunity it is recommend that dose 2 be administered week 8 and 10 after your first dose. To reschedule your vaccine appointment please phone reception 9386 3409
All AstraZeneca appointments will be on a Wednesday only. This due to the high volume to vaccines we have already administer to this age group over the last 8 months

select your name, and then view immunisation history statement (PDF)

Log on to the app
Select Immunisation history from services
Select name, and then select view immunisation history statement (PDF)
