Healthy Heart checks

These are best done annually for those aged between 50-74. It involves time spent with both our nurses and Doctors and these appointments will be bulk billed.

You are required to have some pathology taken.

These results will be analysed and put into the Cardiovascular risk calculator to determine your heart disease risk.

Together a plan can then be developed to help lower your risk factors as required.

Health Assessments

All Health assessments involve time spent with both our nurses and our Doctors and these appointments are bulk billed.

There are two types of health checks,

1. For people aged between 40-49yrs These appointments take 30-45 mins. Criteria applies, please ask for more details

2. For people aged 75 and older. These appointments take between 45-60mins. For best results they are done annually.

GP Chronic condition management plans

These are for people that have chronic illness/s that require a team of medical specialists and/or allied health professionals to assist you to best manage your ongoing conditions.

This is a individual plan made with you by our nurses and Doctors to document your conditions and help plan goals and develop a holistic management plan that involves all of your specialists and allied health with you at the centre.

This plan is reviewed every 3-6months as determined by you and your clinical team.

To assist you in reaching your goals you maybe also be eligible for 5 Medicare subsided Allied health visits per calendar year.

We provide this as a bulk billed service

Menopause and Perimenopause assessments and plans.

These can do done by your GP or in conjunction with both our womens health nurse Felicity and your GP.

This involves a comprehensive assessment and development plan for ongoing management and follow up.

Please call the clinic to book so we can allocate the correct time for your appointments.

We provide the nurse part of this assessment/ plan and follow up as a bulk billed services.
